I 'm really interesting in mahara but i need adapt it.
Do you think i can adapt the register form to allow user to directly create institutions?
Can i create different institutions type? like institutions vs commercial

You probably could adapt the register form to do that, yes. Have a look at the institution page to see how institutions are added (i.e. what data is needed and where it is inserted in the database).

There's no concept of institution "type" in Mahara. You can name them however you want though. What would you imagine would be different between a "normal" institution and a "commercial" one?

I imagine different field between "normal" and "commercial" and i need to separate them. In fact i would make 3 category user, normal institution, commercial institutions with their own template and could be discriminate with a search form
